Strada’s sudden departure comes amid mounting scrutiny over how the execution was handled and whether existing protocols are adequate.

“The Tennessee Department of Correction takes its responsibilities seriously and will continue to review all procedures,” Strada said in a brief statement.

Opponents of the death penalty are already seizing on the incident to push for further delays and legal challenges.
